Documents Required for FSSAI Central License

The exact documents required depend on the Kind of Business, constitution of the entity and food activities performed at the premises.

According to the FoSCoS Central License document checklist, the following documents may be required.

Form B duly completed and signed by the proprietor, partner or authorized signatory.
Blueprint or layout plan of the processing unit showing dimensions and operation-wise area allocation, where applicable to manufacturing or processing units.
List of Directors, Partners, Proprietor or Executive Members, along with their addresses and contact details, as applicable.
List of equipment and machinery showing number, installed capacity and horsepower for manufacturing and processing units.
Identity and address proof of the proprietor, partner, directors or authorized signatory, as applicable.
List of food categories proposed to be manufactured.
Authority letter identifying the responsible person and alternative responsible person for manufacturing or processing operations.
Chemical and bacteriological water analysis report where water is used as an ingredient and such report is applicable.
Proof of possession of premises, such as sale deed, rent agreement or electricity bill.
Business constitution documents, which may include Partnership Deed, Proprietorship Declaration, Memorandum and Articles of Association or other applicable entity documents.
Documents Required for a Food Manufacturing Business

Manufacturing and processing units usually have more extensive documentation requirements because FSSAI needs information about the production facility, machinery, raw materials, water quality and food safety system.

A manufacturing unit may therefore need its premises layout, machinery details, manufacturing food categories, water analysis report, FSMS documentation, responsible-person authorization, raw material information and other category-specific reports in addition to the basic entity documents.

The FoSCoS document checklist specifically identifies the layout plan and machinery list as mandatory for manufacturing and processing units and requires a water analysis report in applicable manufacturing/processing cases.
